Lessons learned

When you read these examples, you may want to –

• Consider - is this relevant for our practice? Could we learn from this?
• Act - if possible, make changes in your practice
• Alert - tell senior staff if changes are needed, but you need their help or support to make them
• Close - ensure changes are completed and practice team know about them
