Primary and Community Care Compendium of Roles and Models

What is it? 

This is a once for Wales platform for the sharing of innovative and unique models of primary and community care provision. It has been produced on behalf of the Strategic Programme for Primary Care (SPPC) in collaboration with Health Education and Improvement Wales (HEIW).

Each case study showcases integrated multi-professional working to provide person centred care, closer to home. Those involved are professionals with lived experience, working in Wales at the top of their licence, in new or innovative ways.  There are links on each page to supporting resources.

How can it be used?

It can be used by anyone developing healthcare services to identify novel approaches to meeting population health needs.  Research and development activity can draw learning from the case studies before commissioning new services to maximise impact on patient care.

As primary and community care evolves, new case studies can be shared via the Submit a Case Study button on the Homepage and Case Studies page.
